The Loyola New Orleans Preceptor Agreement
The Preceptor Agreement is the Loyola University New Orleans form in which your preceptor accepts the role and its duties. Graduate students upload it with the Exxat Wishlist. It sets out orientation, supervision, logs, faculty contact and the final evaluation, and it flags when the preceptor's employer needs its own agreement with Loyola. DNP project practicums use a separate form.

What the form is for
Loyola uses two layers of paperwork for each graduate practicum. The affiliation agreement binds Loyola and the organization. The Preceptor Agreement binds the individual clinician to the work of teaching you, and it is one of the uploads Loyola's Wishlist guide asks for alongside the preceptor's CV and license.
Loyola posts the form on the Graduate Nursing Student Resources page, so you can share it with a prospective preceptor early. Reading the duties up front lets a clinician decide with full information, and it gives you a natural way to open the conversation: here is exactly what Loyola asks of you. Because the duties run to the last day of the term, including the final evaluation, the form deserves a careful read by both of you rather than a quick signature.
The duties your preceptor accepts
The Preceptor Agreement lists what the preceptor takes on across the practicum.
- Orient you to the site and introduce you to key staff.
- Offer a range of learning experiences suited to the course.
- Mentor you and give regular, constructive feedback.
- Sign your practicum log on a regular basis.
- Talk with Loyola faculty at the start and at the end of the practicum.
- Report problems to Loyola as they arise.
- Coordinate your clinical hours with you.
- Evaluate you and submit the Practicum Evaluation Form at the end.
These duties sit on top of Loyola's rules about who may precept at all, from licensure and certification to experience and family ties, laid out on Loyola's preceptor requirements.
The employer clause
One line in the form catches students off guard. By signing, the preceptor acknowledges that Loyola needs a contract with the facility where you train, and a second contract with whoever employs the preceptor if that is someone else.
This matters for clinicians who are contracted rather than employed by the site: an NP paid by a staffing company, a physician employed by a group that staffs a hospital unit, a psychiatric clinician working for a telehealth company inside a community clinic. Each of those arrangements can mean two contracts instead of one. Ask about employment in your first conversation with a preceptor, then read affiliation agreements for how Loyola handles the second contract. The earlier you know, the earlier the coordinator can start that second review.
Where the signed form goes
Graduate students upload the signed Preceptor Agreement to Exxat as part of the Wishlist request, together with the preceptor's CV and license, which for NP preceptors is a Nursys PDF. The Wishlist guide covers each upload and the approval that follows.
Undergraduates follow a different route, and Loyola's documents name a different place for them. Per the Nursing Student Handbook, a BSN preceptor signs before clinical time begins, the student posts the form in the Canvas course, and the clinical schedule goes to the clinical instructor a minimum of four weeks in advance. BSN students should follow the Canvas instructions for their own course. If you are unsure which route applies to you, your course syllabus or clinical instructor will say.
The DNP Scholarly Project Preceptor-Student Agreement
Project practicums in the DNP (NURS G939, NURS G942, NURS G944) run on a separate document, the DNP Scholarly Project Preceptor-Student Agreement. Three people sign it, namely the project preceptor, you and a Loyola faculty member, and one form spans all three project courses.
Its contact rule differs from the NP form: every semester, the project preceptor talks with your Loyola scholarly project advisor at least once, by Zoom or by phone. The DNP letter also requires the project preceptor's license to be unencumbered in the practicum state and bars relatives, employers and direct supervisors from the role. Because one agreement spans three terms, choose a project preceptor who expects to stay in the role long enough to see the project through.
How the agreement plays out during the term
In some tracks the signed form shapes daily practice. For acute care students, the AGACNP Practicum Guide reserves invasive procedures for the preceptor of record, meaning the clinician whose signed agreement sits in Exxat Prism, so ask your course faculty before doing a procedure with anyone else. The guide does let students learn alongside radiology, interventional radiology and ultrasound providers. Evaluation requests go to AGACNP preceptors through Prism at the midpoint and near the end of term.
In Louisiana, state rules sit beside Loyola's form. Under LSBN's preceptor rule, anyone precepting in Louisiana must hold a Louisiana RN or APRN license that is active and unencumbered, and one preceptor can oversee at most two graduate APRN students in the same clinical experience. Questions Loyola students ask
Is the Preceptor Agreement the same as the affiliation agreement?
No. The affiliation agreement is a contract between Loyola and the organization, signed by someone with authority to bind it. The Preceptor Agreement is signed by the individual clinician who supervises you and lists their teaching duties. Graduate placements need both, and some need an extra affiliation agreement with the preceptor's employer.
Does my preceptor get paid for signing Loyola's Preceptor Agreement?
Loyola's contract with sites involves no payment in either direction, and the preceptor form itself describes duties rather than compensation. If a preceptor asks what Loyola offers in return, refer the question to the School of Nursing; the Clinical Practicum Coordinator is the right contact.
When must the Preceptor Agreement be signed?
Before hours begin. Graduate students submit it with the Wishlist, and no practicum time counts until the full request has reached Placement Approved. For undergraduates, the handbook says the preceptor signs before clinicals start and the form goes into the Canvas course, with the schedule shared four weeks ahead.
Who signs the DNP project agreement?
Three people: the project preceptor, the student and Loyola faculty. One form serves all three DNP project practicums, NURS G939, NURS G942 and NURS G944, and it commits the project preceptor to a Zoom or phone conversation with your scholarly project advisor every semester.
My preceptor works for a staffing agency. What changes?
The preceptor form makes clear that Loyola must hold a contract with the site and, when the clinician's employer is a separate company, with that employer as well. Loyola may therefore need a contract with the agency as well as the clinic. Tell the Clinical Practicum Coordinator early, because a second agreement adds review time.
