The Exxat Wishlist: filing your Loyola placement request

The Wishlist is the Exxat form that turns your chosen site and preceptor into a formal placement request at Loyola University New Orleans. It asks for the agency, a contact, the authorized legal signer and the preceptor, and it carries the preceptor's CV, license and signed Preceptor Agreement. Where the Wishlist lives and when to file it

Inside Exxat Prism, open the Dashboard and choose Submit Preferences; that brings up the Wishlist preference form. Loyola posts a 2025 Wishlist guide on the School of Nursing resources page, and the Clinical Practicum Coordinator, Loyola's technical advisor for Exxat, can help if a screen does not behave.

File once you have a real commitment from a site and a clinician, not while you are still asking around. The request starts Loyola's review of the agency, the contract and the preceptor, and a preceptor who declines later sends you back to the beginning. The first four fields describe the organization, not the person teaching you. That difference matters most for the signer: your preceptor may be glad to host you and still have no authority to sign a contract for the practice.

Before filing, measure the clinician against Loyola's preceptor requirements, which cover licensure, certification, practice experience and personal ties.

The uploads that travel with the request

Loyola's Wishlist guide lists the documents you attach alongside the fields. Collect them in one pass, because a request missing an upload waits until it arrives.

  • Preceptor CV. A current resume or curriculum vitae showing the clinician's practice history.
  • Preceptor license. For NP preceptors, the guide asks for a Nursys PDF.
  • Signed Preceptor Agreement. The Loyola form in which the preceptor accepts the teaching duties; see the Preceptor Agreement.
  • Project practicum agreement. DNP scholarly project practicums use the facilitator agreement, the DNP Scholarly Project Preceptor-Student Agreement, in its place.
  • Optional: a site-signed Standard Loyola Affiliation Agreement. If the site has already signed Loyola's standard form, you can attach it.

Training where you already work adds one more form: Loyola's attestation for students employed at the clinical site, which you, your supervisor and your preceptor sign to show that practicum hours are separate from your job in both department and schedule.

The signer and the second agreement

The authorized legal signer field deserves the most care, because it connects to the affiliation agreement. If Loyola already holds an active agreement with the organization, the request can move quickly. If not, Loyola's standard three-year agreement for 2026-27 goes to the person you named, and a site that edits it heavily or sends its own contract can add several months.

Check one more thing before you submit. Loyola's preceptor form points out that when a clinician's paycheck comes from an organization other than the site, Loyola needs a contract with that employer as well as with the facility. A physician employed by a medical group but seeing patients in a hospital-owned clinic is one example. The full contract rules are on affiliation agreements.

What Placement Approved means

Loyola reviews a Wishlist request in parts: the site, the agreement, the preceptor's documents and your own compliance. Some parts can show approved while others are still open. The FNP guide is explicit that hours may not begin until the request as a whole reads Placement Approved.

Your compliance record is part of that picture. Any lapse in requirements can bring a compliance hold that blocks registration for the practicum course, so keep Required Documents in order while the request is under review, and replace anything that expires before the term ends.

Watch the status of the whole request, not its parts. Hours logged before the affiliation agreement carries every signature are forfeited, and the bulletin says the course may have to be repeated.

Settings that need a director's sign-off

Some requests need an approval beyond the standard review. FNP practicums belong in outpatient primary care, so an FNP request that points to urgent care, a hospital ward, an emergency department, a nursing home or a specialty clinic has to clear the Graduate Program Director. For FPCM IV, a specialty site is approved by the FNP Director, and extra specialty hours are requested through a form Loyola keeps on its Nursing Resource Site.

For NURS G829, confirm before filing that the site and preceptor fit FPCM III's population: 150 pediatric and 50 obstetric hours. For acute care courses, remember that the AGACNP guide has you arrange shifts around your preceptor's schedule, so ask about that schedule before you list the preceptor. Where do I find the Wishlist in Exxat Prism?

Open the Dashboard in Exxat Prism and choose Submit Preferences. Loyola's 2025 Wishlist guide on the School of Nursing resources page walks through the form. You reach Prism with Loyola single sign-on, and the STEPS subscription is the one that covers placement processing, so buy it before you try to file.

What license document does my NP preceptor need to upload?

Loyola's Wishlist guide asks for a Nursys PDF for NP preceptors. Nursys is the NCSBN license database, and Loyola already uses it to verify and monitor student RN licenses. Upload the PDF together with the preceptor's CV and signed Preceptor Agreement so the request is complete in one pass.

Do I need the affiliation agreement signed before I submit the Wishlist?

No. Attaching a site-signed Standard Loyola Affiliation Agreement is optional, and the Clinical Practicum Coordinator obtains agreements with sites. Every party must sign before any hours start, though, so if the site is willing to sign Loyola's standard form, getting that done early can shorten the wait for Placement Approved.

Some parts of my request show approved. Can I start?

Not yet. Loyola's FNP guide explains that pieces of a request are cleared one by one, and your hours wait until the entire request carries the Placement Approved label. Starting early puts those hours at risk of being forfeited, and the bulletin says the course may then have to be repeated.

What if my preceptor is a physician or PA rather than an NP?

The Wishlist still needs their CV, license and signed Preceptor Agreement; the Nursys PDF instruction applies to NP preceptors. Check the course first: FPCM I and II allow an NP, MD, DO or PA, physician preceptors should be board certified in their specialty, and FPCM III supervision has to fit its pediatric and obstetric population.
