What the FERPA release authorizes

The form gives Loyola New Orleans your written permission to send three kinds of records to a clinical site: immunizations, your background check and your drug screen. FERPA is the Family Educational Rights and Privacy Act, the federal law that protects student education records, and Loyola's form uses it as the basis for sharing your compliance results with the organizations that host you.

The consent is part of the graduate placement sequence in Loyola's practicum materials. Where the form fits in the Exxat sequence

Loyola's graduate placement process runs in a fixed order, and the FERPA consent belongs near the front of it. Here is the sequence with the release in its place.

1. Start three months outLoyola wants graduate students starting a full three months or more ahead, since the process itself can run that long.
2. Buy the Exxat subscriptionsOne covers access to Required Documents and one covers STEPS placement processing.
3. Upload compliance recordsYour physical, BLS, insurance, immunizations and TB form go into your Exxat profile, where the APPROVE team checks each one.
4. Sign and upload the FERPA consentAdd the signed Consent to Release Student Information to Exxat so Loyola can share your screening and immunization records with a site.
5. Search and secure a siteUse Explore Clinical Locations, confirm the affiliation agreement, then submit the Wishlist with your preceptor's documents.

What the release covers, and what travels another way

The form names three record types: immunizations, the background check and the drug screen. Other items a site may ask for reach it through different routes, and knowing which is which saves back-and-forth.

Your RN license is verified on its own track: the School of Nursing monitors student licenses through Nursys e-Notify. Your preceptor's paperwork (CV, license, Preceptor Agreement) travels with the Exxat Wishlist, not with your release. HIPAA training and similar site extras appear in the FNP guide's list of possible site requirements; the Clinical Practicum Coordinator, who tracks compliance with facility requirements, can tell you what Loyola supplies and what you provide yourself.

One flow runs outside the release entirely. Under the handbook's drug policy, a positive screen goes to LSBN and to School of Nursing leadership. That reporting comes from Loyola's policy, not from the consent you sign for sites. The background check and drug screen guide explains that policy.
